It means the sex you identify as is contrary to the one you were assigned at birth. So, a trans woman was born male but identifies female, and vice versa. Throughout your transition you take hormones, go for surgeries, etc. Transsexual is considered an outdated term, and “transgendered” is actually a misnomer; the correct term is transgender.
